Our Policies

Our Policies2020-06-02T01:17:47+00:00
Evaluation and Treatment2019-06-10T19:28:37+00:00
Beyond the initial psychiatric evaluation, we provide treatment in the form of medication management. We may incorporate psychotherapy (counseling) on occasion as the need arises, but our current providers do not offer formal psychotherapy services on a regular basis.
Letters for Disability2019-06-11T19:12:32+00:00
Please note that as a general rule, we do NOT provide input regarding an individual’s ability or inability to perform their work/school functions. Therefore, we do NOT write letters or complete forms for things such as, but not limited to, disability, FMLA, excuse from work or school (aside from confirmation of date and time that you were seen at the office for a scheduled appointment), and educational/testing accommodations. We also do NOT write letters or complete forms for emotional support animals.
Administrative Fees for Documentation2019-06-11T19:11:53+00:00

Our  fee for producing written letters (from Providers) on behalf of patients, as well as copies of medical records is $40.  This fee covers for the administrative process and resources required for handling such requests, including (but not limited to) both Provider and Staff time, office supplies (such as copy paper, ink, letterhead, etc.) and the communications/transmissions of documents.

Requests for Medical Records2019-06-10T19:37:47+00:00

Please note that our  fee for producing copies of medical records is $35.  This fee covers for the administrative process and resources required for handling such requests, including (but not limited to) both Provider and Staff time, office supplies (such as copy paper, ink, letterhead, etc.) and the communications/transmissions of documents.

Medication Prescriptions and Refills2019-06-10T19:41:08+00:00

Please note that State and Federal Guidelines, as well as those guidelines mandated by our Individual Insurance Company Contracts and our own Internal Regulatory Systems, require a face-to-face appointment with our medical providers prior to prescribing medications and/or medication refills.  This policy applies to both existing and new patients.  No prescription refills are provided via telephone or electronic submission processes – a face-to-face appointment is required.  This policy ensures not only compliance with guidelines with regulatory agencies but also ensures best practices and advocates for patient safety and well-being.

Please note that all Schedule II medications, as required by law, require a paper script to be completed by our medical provider during a face-to-face evaluation and that this paper must be hand-delivered to the pharmacy by the actual patient and/or legal guardian (legal ID is also required by Pharmacy Guidelines).

If needed, please ask our front desk staff for a copy of this policy as well as resources regarding South Carolina Laws pertaining to prescription monitoring and regulatory requirements.

If you are in need of medication refills from one of our medical providers, please contact our office and schedule an appointment.

Prior to First Appointment2019-06-10T19:49:43+00:00

Prior to your first appointment, it is imperative that you provide us with all of the necessary insurance information including your policy number, ID number group number and a contact number for your insurance company. Additionally, it is your responsibility to verify that our provider(s) accept your specific insurance. Specifically, it is your responsibility to find out if your insurance company covers mental health treatment as well as any limitations to treatment and/or services prior to my first appointment. Please bring a copy of the front and back of your insurance card.

Know Your Benefits2019-06-10T19:57:36+00:00

It is your responsibility to know and understand your own insurance benefits which includes and is not limited to having up-to-date knowledge of your co-pay requirements, co-insurance obligations and/or deductible requirements.

Changes with Insurance2019-06-10T19:59:50+00:00

It is your responsibility to notify Sunrise Psychiatry, LLC of any changes to your insurance coverage(s) as they occur. In addition, we expect that you will provide any/all additional information concerning your coverage should a billing problem arise.

Potential Problems with your Coverage/Benefits2019-06-10T20:07:18+00:00

We will gladly file your insurance claims for you on your behalf with the expectation that you will provide any/all additional information concerning my coverage should a billing problem arise.

Please note, that if there is a scenario in which your appointment is scheduled with a provider that does not accept your insurance, it is your responsibility to pay for all of the services you received.

Additionally, in the event that staff at Sunrise Psychiatry, LLC file a claim on your behalf and the fees are not reimbursed due to errors regarding ID numbers, accurate DOB, names, and active/inactive policy dates,  you will be responsible for payment of any remaining balances or unpaid fees.

Prior to Receiving Any Services for Follow-up Appointments2019-06-10T20:15:20+00:00

Payment is due prior to receiving any services for all appointments including initial and follow-up appointments. You are expected to provide full payment for any current or past due balances. Payment due may include but is not limited to, required co-pays, co-insurances, unmet deductibles, self-pay fees as well as including No Show or Cancellation fees.

Co-Pays: For insurance policies that require co-pays, our contract with these managed care companies require full payment at the time service is rendered. We collect this payment prior to you meeting with the provider

Deductibles and Co-Insurance: If you have met your insurance deductible, we will collect only the percentage that you are responsible. If you have not met your deductible, we will collect 100% of the charges at the time of service prior to meeting with your provider

Self-Pay: 100% of charges will be collected from patients that do not have insurance coverage or are having services provided that are not covered by their insurance company.

Bill Forwarding2019-06-10T20:17:13+00:00

Sunrise Psychiatry, LLC does not participate in “Bill Forwarding” processes. This means that the specific patient (or responsible guardian) arriving to the appointment must be prepared to provide full payment for services rendered. Therefore, if a patient arrives without the required payment, we will gladly reschedule the appointment (No-Show fees may/may not apply, depending on the number of occurrences).

Third Party Payments2019-06-10T20:18:07+00:00

If you have arrangements with a third party to provide payment for your services, we require payment at the time of service and will gladly provide you with a receipt for your own reimbursement purposes. You also have the option of leaving a credit care on file at our office for convenient payment options. Please ask a front office staff member to provide you with the proper form.

Cancellation of your appointment2019-06-10T20:21:27+00:00

In the event that you need to cancel or reschedule an appointment, please note that our office has a 24-hour cancellation notification policy. This policy requires that you notify our office via email (help@sunrisepsychiatry.com) or phone (843-800-5070) at least 24-hours in advance of canceling or rescheduling an appointment during business hours, which are Monday-Thursday 8am-4:30pm and Friday 8am-2:30pm. If no one is immediately available to answer your call, please be sure to leave a voicemail and include your full name (or patient’s full name), the provider you are scheduled to see and the date and time of the appointment that you need to cancel or reschedule. This cancellation policy is because when you have an appointment with us, that scheduled time is specifically reserved for you and you alone. We do this to ensure that our patient appointments are efficient and easily accessible without over booking and avoid long wait times. This policy is also maintained to better accommodate other patients who are waiting to be seen and need an earlier appointment.

*Please Note: Voicemails and/or emails received during the weekends or outside of business hours will not be considered sufficient notice as our office is closed, and no one is available to receive or check email or voicemail.

*Please Note: Although we have the capability for you to schedule your own appointments online at our website, sunrisepsychiatry.com, this does not automatically change or cancel an appointment previously scheduled. Therefore, you must contact us directly in order to cancel the original appointment during business hours.

Cancellation / No Show Fee2019-06-10T20:22:59+00:00

If you miss an appointment without calling to cancel or if you fail to reschedule at least 24 hours ahead of time, you will be charged a cancellation fee of $100. You will also be charged a cancellation fee if you arrive late and must be rescheduled for another day. The only exception to this fee is if you can provide documentation you were hospitalized. If you do not come to your appointment without cancelling or rescheduling your appointment (“no show”) you will be required to provide a credit card number in order to schedule another appointment. On the second “no show” or less than 24-hour cancellation, the missed appointment fee will be collected. Please note that although we do offer courtesy email, text message, and phone call reminders, the patient is responsible for remembering his/her appointment. Not receiving an appointment reminder will not be considered an excuse for missing an appointment. Insurance companies do not reimburse for these fees. Repeated missed appointments without adequate notice will result in a review of a patient’s appointment history and may result in a termination of services with our practice.
