New Patients

We look forward to meeting you. Please fill out the online form and someone will contact you as soon as possible to schedule your appointment. Please have your insurance information readily available.

Existing Patients

If you are an existing patient, click the button below to schedule your appointment. Please Note: If you need to reschedule for any reason, call the office directly to avoid being charged a cancellation fee.

Registration Forms

Please note that new registration forms must be completed prior to being seen for your first appointment at Sunrise Psychiatry, LLC. This includes patients who were previously seen by one of our providers at another practice. These forms will also need to be completed at least once a year in order to maintain the most up-to-date information. Please print out these forms ahead of time and bring with you or plan to arrive a little earlier than your scheduled appointment time. This task takes just a few minutes and facilitates our delivery of quality care in the following ways:
